Supplemental Benefits from Symetra

Symetra Voluntary Life


This group term life policy offers coverage for employees in increments of $10,000 up to a guaranteed issue amount of $100,000 and a maximum of the lesser of five times annual earnings or $500,000. Employees who elect coverage under the policy may elect supplemental coverage on a spouse or domestic partner in $5,000 increments up to a maximum of 50% of the employee benefit election.  Child life coverage is available for children under age 26 and the coverage amount is $10,000 for any dependent children of the insured who elects the child coverage (ie. one child or multiple children have the same premium and same per child coverage), the premium for child life is $2.00 / month.

For coverage effective 1/1/2022 Symetra offers supplemental life up to the above listed guaranteed issue amounts without evidence of insurability. Aside from this year's annual open enrollment, the plan will require evidence of insurability for anyone enrolling in or increasing coverage outside of their new hire benefit eligibility period.

Short-Term Disability


Short-term disability insurance replaces a portion of your income if you become too sick or injured to work. Benefit payments for an approved claim will begin once a certain amount of time has passed—called the elimination period—after the start of your disabling condition. You’ll receive these benefits for a limited time—24 weeks—or until you return to work. Accommodations may be made for a gradual return to work. 

This benefit provides 60% of your gross weekly income, up to $700, after an elimination period of 14 days and continuing for a maximum of 24 weeks.

Here is are two examples illustrating how you can calculate the premium cost of this benefit ($0.34 per $10 of covered benefit)

 Weekly Rate  15 * 40 = $600  50,000 / 52 = $961.54

 Premium Amount (monthly)  Amount is $0.34 per $10 of covered benefit.   Amount is $0.34 per $10 of covered benefit.
 Total Premium  (360/10) * .34 =  $12.24 / month   (576.92/10) * .34 = $19.62 / month 


Short-term disability requires evidence of insurability for all new enrollments outside of the new hire eligibility period.

Long-Term Disability


Long-term disability insurance replaces a portion of your income if you become too sick or injured to work for an extended period of time. Benefit payments for an approved claim will begin once 180 have passed—called the elimination period—after the start of your disabling condition. If you have other coverage, like short-term disability insurance, those benefits may be paid for some or all of this time. You’ll receive your long-term disability insurance benefits while you are disabled, until you return to work or reach your plan’s maximum duration (social security normal retirement age).

This benefit provides 60% of your gross monthly income, up to $6000.

Critical Illness Insurance


Symetra's voluntary critical illness insurance provides a lump sum payment if a covered condition is diagnosed after coverage takes effect for the individual. Covered conditions include critical illnesses and/or conditions such as cancer, heart attack, stroke, organ failure, and more (see plan summary). Premiums for this benefit are based on the age of the insured on the effective (start) date of coverage as well as the amount of coverage elected. Employees can enroll in the Critical Illness plan during annual open enrollments or with a qualified life event without need to complete evidence of insurability (EOI).

Accident Insurance


Accident insurance provides benefits to help cover out-of-pocket medical expenses related to an accidental injury. Benefits are paid based on the type of injury or service performed and do not interfere or coordinate with your major medical plan.
